Requests for EHC Needs Assessment can be made by the child's parent, a young person over the age of 16 but under the age of 25, or a person acting on behalf of a school or post 16 institution.

Management of EHC Needs Assessment and review process including decision making re EHC Needs Assessments, writing EHCPs and amendment notices, and consultations with providers re placements. EHCP coordinators are linked to each education provider and provide information and guidance regarding the EHC Needs Assessment and review process. EHCP coordinators also work with parents providing advice regarding the EHC Needs Assessment and Review Process as well as meeting with parents to discuss draft EHCPs. EHCP coordinators also attend some transfer reviews and annual reviews when necessary,

Parents' views are sought as part of the EHC Needs Assessment process and Person Centred review meetings. A child and family feedback form is completed during annual reviews when relevant to gain further feedback from parents.
