The Care Quality Commission is inspecting Northern Lincolnshire and Goole NHS foundation trust in November.
As part of this they would like feedback from people that have used NLaGs services (Scunthorpe General Hospital, Diana, Princess of Wales Hospital, or community services) over the last year.
